What Is Stepped Care?
Stepped care is a progressive model of healthcare that offers interventions in stages-beginning with the least intensive, most accessible support, and progressing to more in-depth evaluations or treatments as needed. This allows for a flexible, person-centred pathway that evolves based on your goals, needs, and circumstances.
Rather than rushing into complex assessments, stepped care recognises that sometimes a thoughtful conversation is the most powerful first step.
Step One: The Initial Neuropsychological Consultation
At the core of our stepped care model is the Initial Neuropsychological Consultation- a two hour session with one of our experienced clinicians. This is a space to explore cognitive concerns, emotional wellbeing, and life changes in a compassionate, structured, and empowering way.
You won’t undergo formal cognitive testing at this stage, and that’s intentional. We begin by listening: gathering your history, your concerns, and your hopes for support. Through clinical interviews and standardised self-report measures, we gain valuable insights that guide what comes next.
This approach ensures that any further assessment is both timely and meaningful-not just clinically appropriate, but also aligned with your unique goals.
Step Two: Cognitive Assessment, if Needed
If both you and your clinician agree that a more detailed evaluation would be helpful, the next step is a comprehensive neuropsychological assessment. This may involve testing memory, attention, executive function, and other areas of cognitive performance.
These assessments help clarify diagnoses, inform treatment planning, and identify strengths to build upon. But crucially, we only proceed when it’s appropriate and welcome-a decision made together, just for you.
Step Three: Therapy, Rehabilitation, or Ongoing Support
Depending on the results and your needs, the third step may involve:
- Neurorehabilitation planning
- Psychological therapy
- Liaison with your wider care team
- Ongoing monitoring and follow-up support
At every stage, the care you receive is collaborative, respectful, and adaptable. This stepped model avoids unnecessary assessments and ensures that each phase builds upon the last.
